My office is always looking for new patients and for past patients to return.  Most skin conditions are able to be treated in one of two visits, and chronic skin conditions often only require an infrequent visit to my office.  

There are patient history forms which only take a few moments to complete.  However, if you wish, my office can mail you the forms to complete in advance.  In such case, you may ask for these forms when you make your appointment.  Patients that have not been seen for over 3 years, also have to complete these forms.
 
Let me also give you a list of items to bring to your appointment.

  • Your most recent insurance card
  • A government issued photo ID
  • A list of your current medications
  • A list of your allergies
  • A list of creams and gels that you have tried on your skin eruption
  • Reading glasses (if needed) as Dr. B often likes to have you read informational sheet on your condition before you leave the examining room

As in all medical fields, the specialty of dermatology requires mutual understanding, respect, and confidence.  We try to offer patients high standards of dermatologic medical care.  If you have questions, or concerns about any part of your care, treatment, or medication, please bring them to our attention.  Should you encounter any difficulties with medications or treatment, contact our office immediately.  Thank you for coming to us for your dermatological care.  We appreciate your confidence.

  • Biopsy of unknown rashes or tumors for pathological assistance on diagnosis
  • excision or electrodesiccation and curettage of skin cancers (basal cell carcinoma and squamous cell carcinomas)
  • Patch testing for determination of cause of allergic dermatitis
  • Incision and drainage of boils
  • Fungal and bacterial cultures
  • Skin tag removal
  • Cryosurgery (or liquid nitrogen) removals

Tuesday: 9:00 - 4:00
Wednesday: 8:30 - 12:00
Thursday: 9:00 - 5:00
Friday: 8:30 - 12:00

In relation to emergencies, if the problem can wait until office hours resume, leave a message of your concern on the normal phone line (419-885-3403).  

If necessary to reach the doctor after office hours, call his cell phone at 419-466-7244.  Remember that for many problems, Dr. Burkhart needs the chart to verify information.
